NDIS Information
1. CONFIDENTIALITY
The privacy of all communication between you and your counsellor is protected by law. Your counsellor may take notes throughout the session, which will then be professional records for the purpose of reference for the counsellor.
Any information can only be disclosed to others by law if:
If a client is threatening serious bodily harm to another or themselves or under the threat of being seriously harmed.
If a court of law subpoenas case notes.
When the client agrees for the counsellor to share relevant information with other professionals, e.g., for referral.
When the counsellor is seeking professional advice from a mental health professional supervisor. Names are not usually revealed in these cases.

Support Provider: Nurture Our Nature
Counselling Professional: Hilke Legenhausen
Service Request Nurture Our Nature Counselling services specific to identified goals (key
focus areas and specific goals to be determined as part of the first meeting).
Service Provided:
· Initial Consultation and brief email (to form a base line for treatment services)
· Ongoing counselling services
· Session duration: 1 hr ongoing counselling services, depending on the goals and needs, comprising of 50 min face to face session, session planning, documentation, and summary email and action recommendations (if required).
Email Summary: Completed within 2 working days of the session, if required.
4. OUTCOMES AND MILESTONES (expected during this service plan)
Support Type: Counselling
Outcome / Milestone: goals to be agreed on in initial counselling session and frequently revisited to ensure that the sessions are on track and congruent with NDIS goals.
5. ESTIMATED SUPPORT HOURS AND COSTS
All prices for therapy sessions are in accordance with the Price Guide for NDIS Service Providers.
The NDIS will update prices on at least an annual basis, effective 1 July each year. Participants will be
advised of any price changes during the period of this service agreement.
The gazette rate for counselling services is $156.16 per hour, the rate of which Nurture Our Nature Counselling is applying to all services is $156.00 per hour.
Travel also may be charged for services provided. Participants will be advised of the cost of any travel to
be claimed from their NDIS plan as well as any changes to travel during the length of their plan.
Wherever possible clients are grouped according to location and the travel costs are apportioned across the number of clients seen at that location.
NDIS Service Agreement and Plan –
The below is an outline of what services might look like, however the below does not indicate exactly what services will include.
Payments will be made:
Self-Managed clients will pay Nurture Our Nature Counselling into their nominated bank account, as indicated on the invoice, or provide Plan Managers details for invoicing.
SUPPORT TYPE as per current NDIS funded plan –
Improved Daily Living Skills - Individual Counselling - support item 15_043_0128_1_3.
Sessions consist of counselling + 10 mins for notes and preparation (50 min session, preparation, documentation, email summary if required. Travel time and sessions longer than 50 minutes will incur additional charges according to the NDIS schedule.
Nurture Our Nature Counselling will charge for time taken for all services however a minimum of 1hour, servicing will be invoiced for all sessions.
Additional BILLABLE TIME:
All the below additional services will be billed at $156 per hour.
· Extended Treatment Session. At times a 50-minute session may not be adequate to cover the
· required tasks. If the session is longer, this will be discussed with you and invoiced accordingly.
· Lengthy phone-calls and emails (over 10 minutes)
· Creating or sourcing resources specifically for a participant (e.g. programs, social stories)
· Attending meetings/liaising with other professionals
· Any letters of support as required
· Any other time directly related to a participant's clinical care/goals
“A supply of supports under this Service Agreement is a supply of one or more reasonable and necessary supports specified in this statement of supports included, under subsection 33(2) of the National
Disability Insurance Scheme Act 2013 (NDIS Act), in the participant’s NDIS Plan currently in effect under section 37 of the NDIS Act.”
IMPORTANT NOTE:
Regardless of how the NDIS plan is being managed, if an invoice is unpaid because a participant has exhausted the funds in their plan, they will be personally responsible for paying that invoice.
Additionally, Nurture Our Nature Counselling will budget services based on the total amount indicated above. Should you choose to re-allocate capacity building funding resulting in this above amount not being available, services will not be completed.
CANCELLATION POLICY:
Please contact Hilke on 0432 469 695 as soon as possible if you are not able to attend your appointment or need to reschedule.
If you are concerned about your ability to reliably attend appointments, please discuss this with Hilke so that the most appropriate time can be determined for the appointment and support mechanisms implemented to help you to remember appointment times.
Nurture Our Nature Counselling requires a minimum of 48 hours’ notice of changes to, or cancellation of, appointments.
· Less than 24 hours’ notice will incur an 100% service fee cost
· 24-48 hours’ notice will incur a 50% service fee cost
The NDIS price guide states that “Where a provider has a Short Notice Cancellation (or no show) they are able to claim 100% of the agreed fee associated with the activity form the participant’s plan, subject to the Price Guide and the terms of the service agreement with participant”.
INVOICE PAYMENT
Nurture Our Nature Counselling will provide you or your planner with the invoice within 24hrs of the service.
Payment of an invoice is expected prior to the next appointment.
Failure to pay for the servicing will result in services being suspended. If this cannot be resolved, you
may be declined future services.
Nurture Our Nature Counselling’s only payment option is direct deposit into our bank account.
